Four no-prep feldspathic porcelain veneers were used on the upper lateral incisors and canines to improve proportions and smile harmony while leaving the central incisors untouched.
The Patient’s Concerns
This patient came to Newport Beach Dental Studio while preparing for her wedding and wanted to improve the overall harmony of her smile without undergoing unnecessary dental treatment.
She felt that her two central incisors visually dominated the smile. She was also unhappy with the warmer color of her teeth and with the shape and wear of her upper lateral incisors and canines.
The upper right lateral incisor was also mildly retroclined, creating a subtle alignment discrepancy. Importantly, however, she was happy with the central incisors themselves and did not want them altered.
The challenge was therefore to improve the overall proportions and flow of the smile while treating as few teeth as possible.

Why We Left the Central Incisors Alone
Although the patient’s central incisors appeared visually dominant, she was happy with those teeth and there was no reason to place restorations on them simply to create a conventional six- or eight-unit veneer case.
Instead, we changed the proportions of the teeth surrounding them. By increasing the visible dimensions of the lateral incisors and refining the worn canines, the relationship between all six anterior teeth changed even though the two central incisors remained completely natural.
This is an important principle of conservative cosmetic dentistry: treat the source of the visual imbalance rather than automatically treating every tooth visible in the smile.
Four Veneers Instead of Six
Cosmetic cases involving only four anterior veneers require careful case selection because the new restorations must blend directly with natural central incisors while also integrating with the posterior smile.
In this patient, the position and appearance of the central incisors made a selective four-veneer approach possible. Feldspathic porcelain allowed the lateral incisors and canines to be modified conservatively while preserving the natural teeth she already liked.

Analyzing the Smile Design
Improving Lateral-Incisor Proportions
The lateral incisors appeared relatively small compared with the central incisors. Increasing their apparent dimensions created a more gradual transition across the anterior smile and reduced the visual dominance of the centrals without altering them.
Correcting the Retroclined Lateral
The upper right lateral incisor was mildly retroclined. Because the discrepancy was limited and the case was favorable for an additive approach, the facial contour of the veneer could be used to improve the tooth’s apparent position within the smile without orthodontic movement.
Restoring the Canines
The canines showed wear and less ideal shape. Restoring their visible contours helped establish a smoother transition from the lateral incisors toward the posterior teeth and improved the overall framing of the anterior smile.
Preserving the Natural Centrals
One of the most significant aspects of this case is what was not treated. The patient’s natural central incisors remained intact. The surrounding restorations were designed to harmonize with those teeth rather than requiring the central incisors to be prepared simply for symmetry of treatment.
A More Harmonious Anterior Progression
The final smile demonstrates a more balanced progression from central incisors to lateral incisors to canines. The teeth no longer appear as visually separated groups; instead, their proportions and contours create a more continuous composition across the smile.
Why Feldspathic No-Prep Veneers?
The lateral incisors and canines primarily needed additional contour and volume rather than aggressive reduction. That made this case suitable for an additive no-prep approach.
Thin feldspathic porcelain can transmit aspects of the underlying tooth color and provides the ceramist with considerable control over translucency, surface character and light reflection. Those optical characteristics were particularly important because the four restorations needed to blend directly into untouched natural central incisors.
Not every patient is appropriate for a no-prep approach. Existing tooth position, restorative space, bite and the desired changes must all be evaluated before choosing this treatment.
KöR Whitening Before the Veneers
The patient wanted a brighter smile before her wedding, so KöR Whitening was completed before the definitive feldspathic veneers were fabricated.
She whitened at home for approximately two weeks. After whitening, time was allowed for the tooth color to stabilize before updated photographs and records were obtained.
Those updated records were especially important because the master laboratory technician needed to match the four porcelain restorations directly to the patient’s natural central incisors and surrounding whitened dentition.
Digital Planning & Ceramic Matching
The case was documented digitally with scans and clinical photography. These records were used to communicate the desired tooth proportions, contours and optical characteristics to the master laboratory technician.
After whitening and color stabilization, new photographs and updated records were obtained so the definitive feldspathic ceramics could be matched to the patient’s current natural tooth color rather than her pre-whitening shade.
When only four teeth are being restored within an otherwise natural smile, accurate communication of value, translucency, surface texture and contour becomes especially important because there is little room for visible mismatch.
Treatment Sequence
01 — Cosmetic Consultation
The patient’s concerns, wedding timeline and desire for a conservative smile enhancement were evaluated. The goal was established to improve the lateral incisors and canines while preserving the central incisors she already liked.
02 — Digital Scans, Photography & Planning
Digital scans and clinical photographs were obtained to document tooth position, proportions and restorative space and to plan the four additive restorations.
03 — KöR Whitening
The patient completed approximately two weeks of at-home KöR Whitening. The color was then allowed to stabilize before the final ceramic matching records were taken.
04 — Updated Records for the Master Ceramist
New photographs and updated color records were obtained after whitening so the feldspathic veneers could be matched directly to the patient’s stabilized natural tooth color.
05 — Final Veneer Delivery
Four no-prep feldspathic veneers were delivered on the upper lateral incisors and canines. The delivery appointment required approximately two hours, including evaluation, bonding, cleanup and final clinical photography.

Conservative Dentistry by Design
The objective in cosmetic dentistry is not necessarily to place the greatest number of veneers. The objective is to identify the smallest amount of treatment capable of accomplishing the patient’s goals while respecting healthy natural tooth structure.
In this case, four additive restorations changed the perceived proportions of the entire anterior smile while allowing both natural central incisors to remain untouched.
This type of treatment requires careful case selection, precise planning and close collaboration with the ceramic technician, but when the clinical conditions are favorable it can provide a significant esthetic change with very limited intervention.
The Final Result
The finished smile has a more harmonious relationship between the central incisors, lateral incisors and canines while preserving the two natural central incisors the patient already liked.
The laterals now have greater visual presence, the canine contours are restored, and the mildly retroclined lateral appears more integrated into the anterior smile.
